ISKO-larship for ENGINEERING STUDENTS

As part of the Engineering Student Council’s commitment to promoting student welfare and academic excellence among our fellow students, we bring you the ENGINEERING SCHOLARSHIP EXPO. A relevant project and timely service, this program aims to generate opportunities for students who need assistance and support in their education.
Below are instructions in applying for the scholarship.
1. Choose a scholarship that best suits you.
2. Printout the application for scholarship grant form. Click here to download the .zip file. Contains two files.
3. Fill out the form and attach a 2”x2” ID photo.
4. Prepare the basic requirements.
a. Current Income Tax Return and/or W2 of parents;
i. If applicant is working, his/her Income Tax Return and/or W2
ii. If married, attach ITR and/or W2 of spouse
iii. If income is derived from business, attach income statement
iv. If exempted from filing, attach BIR Certificate of Exemption
v. If parent is unemployed, attach notarized affidavit of unemployment
b. Birth Certificate
c. For freshmen:
i. Form 5 or Certificate of Admission;
ii. Certification of Good Moral Character from former school
d. For students already enrolled in the university:
i. Current Form 5;
ii. True Copy of Grades/CRS Printout;
iii. Certification of Year Level Standing from college;
iv. Certificate of Good Moral Character from the College AND from the Student Disciplinary Tribunal;
v. Certification from the College indicating the remaining units and that the student is not on maximum residency (MRR);
e. For graduate students:
i. Transcript of academic records;
ii. Program of study;
iii. notice of admission (if new graduate student)
f. For Pasuc, Presidential, Leadership, Tuition, Foreign, etc.
i. Letter of recommendation from the head of the institute
ii. Certification of study leave with pay
iii. Nomination from home government
iv. Letter of financial support
v. Letter/s of recommendation from former professors
vi. Recommendation/Certificate of good moral character from current employer/s
vii. List of accomplishments arranged by levels, i.e. in high school, college and community duly authenticated by the appropriate authority
5. Pass all requirements to the Office of Scholarships and Student Services, Rms. 301 to 306, 3rd floor, Vinzons Hall, UP Diliman, Quezon City.
6. Wait for further announcements regarding the interview.
Deadline for filing of applications: June 15, 2011

The basic requirements for the scholarships above are as follows:
1. One photo 2” x 2”
2. Income Tax Return and W2 of parents. If applicant is working, his/her Income Tax and W2, if married, also ITR and W2 of spouse (if Income is derived from business, attach Income Statement). BIR Certification of Exemption if unemployed;
3. For applicants already enrolled in the University, please submit also the following:
a. Form 5 (current)
b. True copy of grades (CRS copy)
c. Certification of year level standing (i.e., 1st year, etc.)
d. For Graduate students: transcript of academic records, program of study;
4. Certification of good moral character from the College and Student Disciplinary Tribunal (Rm. 202, Vinzons Hall). (From former school if incoming freshman);
5. Photocopy of Birth Certificate
